As for your concern(s):

— JotForm accounts can be upgraded to enable HIPAA compliance so the forms created under it will become HIPAA compliant.

— Although we have form templates that you can use, you also have the option to create your form from scratch and customize it to your preference. Depending on the layout that you will be using for your forms, you can also inject custom CSS codes to your forms to style your form aside from the other format and style settings available.

— When it comes to Analytics, we have our own Form Analytics (https://www.jotform.com/form-analytics/). Or, you may also find any of these widgets from the Analytics category helpful — https://www.jotform.com/widgets/category/analytics

— Sending form emails to multiple email addresses is also possible with JotForm. For more information, I would suggest that you refer to this guide — Send-Notifications-to-Multiple-Recipients

— There are many options you can choose from to embed your form to other platforms, however, I'm afraid embedding the form in an email may just result in a conflict that is based on how the email will render the form elements. But, if you are looking to pass form submission data to Constant Contact, we have an available integration. How-to-Integration-Form-with-Constant-Contact

— We have a lot of different payment processors you can integrate your form with and Stripe is one of them — How-to-Integrate-Stripe-with-your-Form

— Setting up conditions in your form is possible with JotForm. We have different conditional logic that you can use — Smart-Forms-Conditional-Logic-for-Online-Forms

— Our forms are mobile responsive by default

— I'm not sure if you have any specific requirements for limiting submissions, but we also have such a feature.

— Aside from you can set up email notification in your form so you will receive an email every time your form gets successfully submitted, JotForm also has a mobile app that you can use so you can get notifications whenever you get a submission.

— You can access form submissions on its own form submissions page which is user-friendly. You can create reports, download the data, or also pass the data to other platforms such as Google Drive, Google Spreadsheet, Dropbox, etc, via integrations.

1. You said you could integrate with Constant Contact, would you be able to do the same with Drip?

2. What is the price for the top-of-the-line JotForm with HIPAA compliance, upgraded for maximum security?

1. We do not have a direct integration with Drip. However, it is possible to connect JotForm and Drip via Zapier - https://zapier.com/apps/jotform/integrations/drip

If you would like we can send a request to our developers for direct integration. However, please note that we would not be able to provide you the exact time when and if this would be implemented. 

2. HIPAA is available for the Silver and Gold plans. We have monthly, yearly and bi-yearly subscriptions. You can check the account limits and prices on our Pricing page.
