Can I connect multiple forms?

    I am a school administrator.

All student applications / emergency contact information (ie. parent emails) are received via Jotform.

I want to set up an approval workflow to get Parent Signatures after a first aid or behavior incident. (Sample report in the URL below)

Can I have two forms communicate data to one another so that if teachers / staff fill out an incident report related to the student, then Jotform autopopulates parent emails and sends approval / Signature requests to the parents (without the staff members filling out the parent emails each time)?

    Thank you!

    Hi,

Would it be possible to have the parents signature on the same form?
This way it would be possible to hide and show teacher/parent sections depending on who is filling out the form, and it's also possible to send emails automatically to require signatures.

For example, if you have a Excel sheet of the parents information, you can use Spreadsheet to Form widget to autopopulate the parent name/email/other info on the form.

When the teacher submits the form, an email requesting signature is sent to the parent. This can be done by sending an Autoresponder email:

The parent receives an edit link to the form that they can open from the email, and when they open the form, they will see only the necessary fields for them.
This can be done by conditional logic:
