A Bunch of Questions

I am in the final stages of evaluating Jotform as a solution for my physician practice. I've got a bunch of questions.

1. I need a HIPAA secured solution. I'll be going with the gold or silver plan. What is the process for executing a BAA and is there an additional charge for that?
2. How do I enable access control to the forms? I'd like to only have our users have access.
3. Is there a way to have the form record who entered the data in the form?
4. I am going to use the tables function as a solution for creating delay reports. Is there a way to have the table filter to yesterday or the last 36 hours? I couldn't find any custom filters to accomplish this. The goal is a physician logging in today would always want to see yesterdays physician's notes.

For reference the two forms I am using are:

https://form.jotform.com/221434915207148 - Secondary form. Receives the data from primary form, records it to a clean table, provides options for next steps.
