Where do the files go and how do I know when i'm approaching my upload limit. I'm considering using Jotform to create a referral form for a dentist office. There is a need to have up to 5 images uploaded with the other form data. Where do those images go? How are they retrieved by the form recipient? Are they sent as an attachment with the form data email?

1) If you want to attache a file to your form autoresponder, please see this guide: https://www.jotform.com/help/393-How-to-Attach-a-File-to-Autoresponder-Emails, there you can attach a zipped file that will contain the images.
2) If you want form fillers to upload images through your form, you can use upload fields:
On free accounts, the available space limit is 100MB. You will be notified, as well you will see a warning message in your account, when you are about to reach the limit:
The images are stored in JotForm servers, you can access them through the Submissions Dashboard in your account: https://www.jotform.com/help/269-How-to-View-Form-Submissions, also through email notifications: https://www.jotform.com/help/25-Setting-Up-Email-Notifications
