Procedure Note corrected
A 20 gauge intravenous cannula was secured in the dorsum of the right hand . The patient was positioned prone on the fluoroscopy table . Hemodynamic parameters - Heart rate, blood pressure and pulse oximetry was initiated and monitored. Intravenous antibiotic, Inj. Taxim 1 gm was administered. No untoward side effects was noted. The area was scrubbed and draped. Fluoroscopy guidance was used for the procedure.
The 11th and 12th thoracic vertebrae were identified in the antero-posterior view. The fluoroscope was tilted obliquely to the ipsilateral side to just view the anterior border of the vertebra on the same side. The costo-vertebral junction and the pleural edge was noted. Skin infiltration with 3 cc of 1% Lidocaine was given. The needle was introduced coaxial to the fluorscopy at the lateral edge of the vertebral body first at the D11 Vertebrae on the left side. Needle was then advanced in close proximity to the vertebral edge to avoid any pleural involvement to avoid patient coughing or pneumothorax. The flouroscope was oriented to view the lateral profile of the vertebra. The needle was further advanced. The end-target was accepted, when the tip of the needle was at the junction of anterior 1/3rd and posterior 2/3rd on the lateral view. Then 3 mlml of Omnipaque 250, a contrast, was injected to confirm the position of the needle was not intravascular( with a DSA confirmation ). Similar procedure was repeated with D12 vertebra on the same side.
Patient was comfortable throughout the procedure. The patient was shifted back to the recovery area and vitals were monitored for the next hour. Pain was assessed at frequent intervals and recorded.
