It seems that your are able to add the headers to be hidden in the preview before submit page.

Unfortunately, there is none, however, may I ask why you need to download during this stage?
Please note that once the form is submitted, you can create an autoresponder email and the user will be able to receive a copy of their submitted data.

In addition, you can customize a PDF Submission Report and attach it in the emails.

The form is create for all "parents" in childcare to fill up certain info
and booking of timing etc. It is difficult for me to create one by one with
their email for the autoresponder. I tried, but it keep sending back to my
own email even though i use another mobile number.

4Well, you can simply create a PDF document and attach it to your autoresponder. That way, when the form is submitted, it will send a copy of the submissions as PDF to your form responders.
You can also add a print button to your form: Print-Button-on-Forms

I still do not get it. Meaning i have to do it manually? Meaning i have
200 parents response, i will need to attached 200 times? Maybe can you help
to show steps by step through video?
Furthermore there is no email required by the person filling up the forms.

This is not what we wanted as now majority of the responder complete the
form on their mobile, outside of home etc. Also it is convenient instead of
printing it out as it is not an important document to print. We want to
have environment friendly and paperless.

If you want to send a PDF document that will contain submitted data to your users via Autoresponder Email, you need to add an email field to the form, create an Autoresponder Email, and set the PDF Attachment option to YES inside of the Autoresponder Email settings. You need to do this only once.

Setting up an Autoresponder Email
Once you create it, go to the Advanced tab and set The PDF Attachment option to YES.
Another option is to add the PDF Document download link to the Thank you page.

Yes, you need an email field on your form so that client who submits and inputs their email into the email field get the auto-response notification.
