(“SLS”) are true, accurate, and complete, (2) Applicant will immediately notify SLS of any change in such financial condition, (3) Applicant waives and releases any claims against SLS, Recipients, and any information-providers arising from any act or omission relating to the requesting, receiving, or release of information, (4) each Applicant represents that he or she is authorized to sign this form on behalf of Merchant, (5) the Applicant agrees to receive calls, texts, emails or mail from SLS or Recipients, (6) Applicant authorizes SLS to transmit this application form, along with any of the foregoing information obtained in connection with this application, to any or all of the Recipients for the foregoing purposes, and (7) Applicant consents to the release, by any creditor or financial institution, of any information relating to Applicant, to SLS and to each of the Recipients, on its own behalf. Furthermore, Applicant hereby waives and releases any claims against SLS, all Recipients, and any information‐providers arising from any act or omission relating to the requesting, receiving or release of the information obtained in connection with this application. This authorization shall be valid for one hundred twenty (120) days unless revoked in writing by Applicant

SLS and each of its representatives, successors, assigns and designees including third party lenders (“Recipients”) are authorized to request and receive any investigative reports, credit reports which may include comprehensive credit histories or hard credit pulls, statements from creditors or financial institutions, verification of information, or any other information that SLS and Recipients deem necessary.
