Multiple attendees - looking for event registration template

    I created my event registration form, but am interested in being able to register more than one attendee using the form.  There may be a contact person that is different from the attendee, and that person may also be registering more than 1 person.

    Is there a template that has an option to add an additional attendee?

    is there a way that I can get help customizing my form?

    What I need on my form is the ability to register multiple attendees and each attendee pick a registration type/fee, then I need to calculate that fee for all registrants to charge via payment method.  

    I also need the option for the attendee (person filling out the form) to choose to Submit Payment By Mail -- print the form -- showing instructions to mail with payment.

    I need to record the attendee(s), their contact information and their registration type.  

    I also need to be able to download the data once it's recorded.  So, all attendees would need to be recorded in the registration database, not just the person that paid.

    I don't know that what I am looking for is possible given my limited experience.  

    Maybe it is just best if I only let one person register at a time. 

    Any suggestions would be appreciated.

    You can check for templates at and select registration then add Payment Integration in it.

    We use Conditional logic or Form Calculations

    then we assinge Calculation-Value to each registration type.

    For the option for payment by mail, you can use Purchase Order App.purchase_order_app
