Thanks for reaching out to Jotform Support. We are sorry that you are experiencing this issue. Rest assured that we are here to help you with it. After more checking, it appears your account was suspended due to collecting credit card information. As mentioned in our Terms of Use, it is not allowed to collect credit card information using our forms.
If you needed to collect payment, please use one of our available payment processor.
For your account to be unsuspended, we'll need to delete the offending fields from your form. Would you allow us to delete the offending fields so we can reactivate your account?
