I signed a contract for wedding videography on Jot Form and it's been over 6 months and still no video. Contract said 4-6 months. I cannot find my copy.

Thanks for reaching out to Jotform Support. It seems you filled out a form created by a user in Jotform. You may contact the form owner directly by replying to the Autoresponder Email that you received when you submitted the form. If you are not able to find the email, you can submit the form again and reply to the Autoresponder Email you received to contact the form owner directly.
