6 Online Forms for Human Resource Professionals

As the organizational driving force behind a company, human resource professionals need to be fluent in online forms. They should become familiar with an online form building software like JotForm, and know how to whip up an online form on the fly and keep track of data. Here are some types of online forms that you can create to help your company streamline its workflows and collection of necessary information. 

1. Job application forms
Screening candidates for experience and fit is a top challenge for HR. The job application form is typically the first point of contact between a recruiter and a candidate. You’ll want your application form to collect a resume and perhaps an optional cover letter, but you may want to customize the questions as well. Specific to the role, your form can also ask for relevant information such as their Github URL, LinkedIn profile, portfolio link, or anything else that can aid your team in screening candidates for particular roles. 2. Leave request forms
Request for leave or approved absence forms are important in certain personal circumstances for employees, such as care for a family member, a family member’s bereavement, or other compensatory time off. The form should collect necessary information such as reason for leave, and time frames that the employee is requesting off. If it’s the protocol for your company, you can also collect an employee’s or supervisor’s digital signature straight from your time off request form using an easily added widget.

3. Employee award form
Does your company have an employee of the month program, or does it honor individual employees for their contributions at an annual company party or retreat? An employee award form can collect poll votes or nominations of employees, and can be done anonymously if desired. Recognizing employees for their accomplishments can improve morale and give employees of distinction the credit that they deserve. 

4. Employee accident report form
Hopefully employee accident report forms never have to be used, but when an unfortunate incident occurs, it’s best to be prepared. This form can collect critical information, including the names of the employee(s) involved, the description of the incident, witnesses, and injury and property loss information. Every human resources team should have an employee accident report form within reach, just in case. 

5. HR complaint form
A company should make it easy for employees to speak up about any negative and harmful experiences that are occurring at the workplace. The employee complaint form should allow employees to select the type of situation (harassment, abuse, etc.), describe the incident(s) affecting them, names of those involved, and any relevant dates. The complaint form can provide a link to the company’s formal policy, and when they can expect to receive a response. 

6. Performance review form
Performance review forms, whether given out quarterly or yearly, can be a tool for reflection and communication. The thought of performance reviews doesn’t have to elicit groans and scorn - it can be a valuable way for employees to provide feedback. It can even be a private exchange between mentor and mentee as a way of keeping track of progress and encouraging professional growth.
