How do I do this. My form has 11 pairs of fields (husand and wife name) and it is very long to look at it in a single column. Can I create a way to order the fields in two separate columns? I have tried moving/up/down/newline and am not going to able to do it that way.

Would you like it to have to columns like this:
Select the field and click on the gear icon, then click on Merge to above line:

Excuse me, I know I said this is what I wanted but I was mistaken. I already have this but actually want 5 columns.
1st col 2 col 3 col 4 col 5 col
Attendee name, spouse name attendee name, spouse name
first 2 columns name & spouse
3rd column blank space
4 & 5 col name and spouse

You can alter your forms field positioning by following this guide here:

You should be able to line up your fields as you'd like.
