How do I add a simple 2 box quick cost estimator?

    I am not wanting a form that has to be submitted and I do not want to redirect users to another form or page as I am not selling anything.  All I want are 3 small horizontal boxes where box 1 is the estimated mileage and box 2 is the estimated time.  Then I need box 3 to multiple box 1 by .40 and box 2 by .20 and add them together to instantly give the customer a quick quote.

     

    I have been playing around with the Form Calculation Widget and when I preview the form, it works great (except I can't get it to show in $, just whole numbers) but when I add the form to my site, it no longer calculates the price any more.  Here is the form:  http://form.jotform.ca/form/42055834523250 But I don't want it to open in another window, I want them to be able to get it from my home page.

     

    I tried to send a screenshot but now that I have been playing around with it, I can't even get it to go back horizontally, now it wants to stay vertical but at least it will give you the general idea.  Oh, and ignore the page, I am still working on it, lol

    I have cloned your form and recreated the calculation field to fix positioning issue and add a $ sign.

    It seems that the form source code on your web page is being affected by other website scripts and styles. Can you try to use an iFrame form code instead? The iFrame should not conflict with other content.
