Hi,
I believe this is because of the settings in your "Preferences".
Could you set the "Responsive Form" to "No", only in the "Preferences", leave the rest as it is.
Do let us know if that works.

But I need the form to be responsive. Is there no other solution?

You can achieve that by Injecting Custom CSS. To do that inside of your Form Builder select Setup & Embed tab, then click on Preferences, then select Form Styles tab and inside of Inject Custom CSS field at the bottom of existing code paste this code:
#lite_mode_13,#lite_mode_12{
width:82px!important;
}
@media screen and (max-width: 480px) {#input_12_pick,#input_13_pick{
margin-left:45px;
}
}
After adding this code your form should look like this on mobile phone. And form will be still responsive.
