Form is Auto-Filled with Previous Information

Also, another weird thing that its doing that I'm unsure how to fix (and doesn't do on my other forms on the site). I have gone to the webpage with the form and tried filling it out to test the form myself, putting in a fake name, email, etc. Every time I go re-visit the form on the webpage, the form already has those previously filled out fields completed... even if i refresh the page. I have no idea why its doing this, i don't know if theres a setting I used when creating the form but I want it to be blank everytime I come back to the page and not filled in already. 

I have cloned and tested your form and I saw that you have Enabled Autofill option.

If this option is Enabled it will make the form to autofill with the last updated info.

If you want that form is blank every time you come back to the page you need to Disable Autofill option.
