Hello, I have a similar problem where my form is working on pc's and apple iPhone's but not on any android device. I did not have this issue before. The form can be found at the following link;

Could you please add a screenshot of the issue that you are experiencing and add any specific steps that are needed in order show the issue.
Is the issue shown on the screenshot above? If so I see that you already have a thread about that: http://www.jotform.com/answers/570077 so I would suggest following back on that thread instead.

I have but I did not get any response. I sent this link to many people and theya re having issues accessing it on their android phone. Any suggestions?

OK, I have marked that one as answered so that my colleagues do not look into the issue there while it is being looked into here as well.
Please note that we are assigned to the tickets as they are updated, but it is always best to keep things in a single thread - in order to resolve the issue properly - and much faster.

Can you please check both of the links in your Android Phone - but not by typing them, but by clicking on them.

I've tested your form using my phone (Android 4.4.2) and it loaded correctly.
Have you tried to clear your browser's cache? Can you also let us know which version of Chrome/Android are you running?

The problem is it works on some and others get the 404 page not found. I do not know which browser it is as I have too many people saying that there is the same issue. I never had this problem with this form before. Is it possible it is because I changed the name of the form?

It is very unlikely that changing the form name will cause this issue. I also tested your form in my android device and I am unable to replicate the issue you are having. Your form seems to work as expected. Please check the screenshot below:
This is very strange that it is not working in your device along with your other users as well. I am not sure what is causing this issue. Can you please try the following URL: http://www.jotformpro.com/form/51243797448970
