Hello dmawyin,
I did check your form's notification email alert and did not find any issue with that. You have configured it correctly and you should be receiving the submission email without any problem.
I would suggest you to please speak to your email service provider and ask them to whitelist our domain names to ensure the delivery of emails. Please click on the following URL to get the list of domain names which needs to be whitelisted: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses
You may also like to use your own email address to send submission emails. You will have to add Custom Sender Email in your account and set it as "Sender Email" of your form's email alert. The following guide should help you:

Another alternative is to use Mandrill to send submission emails. You may also like to check the following guide as well: http://www.jotform.com/help/236-How-to-use-Mandrill-to-Send-Emails-From-Your-Own-Email-Address
Hope this helps.
Do get back to us if the issue persists.
Thank you!
