Formatting issue on mobile device

We are having issues with the form not formatting properly within both our mobile and desktop Squarespace site. I copied and pasted the code as instructed. As you can see from the pictures I'm uploading, on the mobile site, the form is shifted to the right side of the page, in front of the rest of the page. On the desktop site, users are experiencing the form cut off on the bottom resulting in no Submit! button. Please advise.

I have added a Mobile Responsive widget to your form, please re-checked your embedded form.

The form seems to be resizing properly on my desktop, if the height issue still persists, please open a separate thread so we will be able to address it properly.
