How to customize an order form

HI I am trying to prepare an order form. Can some fields be removed (lijke the middle name for example) and also I want to add stripe and it says 'only one payment type accepted) so is it in there already? thank you for your help

If this is the form that you refer to, yes, you did already add a payment field which is PayPal. Please click the payment field and remove it as shown in the screenshot:

Please note that you can keep the products on this particular field to be later used on another payment field. You do not necessarily recreate the products again:

Please click yes when this window pops up. It will appear when you hit the remove button that I have shown above. 

About the middle name issue, you can simply do this by clicking on the full name field first:

1. Click on its properties as shown in the screenshot:

2. Then click on the "Options" tab and disable the middle name option:

This should resolve the issue.
