Fillable PDF Errors when required fields are not completed

We use Jotform for performance reviews. Last year we made some fields in the form required and when they were not completed and someone hit submit it created an error in the reporting feature.  

For example the results showed up like this:

What is the form that you turn into a fillable PDF? If you uploaded the fillable PDF to your site, please share the URL where it is located, so we can test it. Or just indicate us what is the form, so we can create the fillable PDF download it in our end and test it.
