I can not find my Thank You page

I had created a thank you form for people who signed up for my FREE Tales2Inapire e-book sampler, but I no longer see that form or a way to reach it if I want to edit it. It contained separate bars - one linking to the Amazon free e-book sampler and one to the one posted on B&N. 

Form appears at bottom of each blog, including the one: http://tales2inspire.com/inspiring-story-blue-heron/

I have checked your account and your form named "Get a FREE Tales2Inspire e-book of Inspiring Stories".

I have seen that your Thank-You page has been set in your Form Builder as you can see the following screenshot:

Also, I have tested your form on your website and I have seen that your Thank-You page has appeared as the screenshot below:

All in all, I am able to view your Thank You page and as I have checked the links work properly as you have already mentioned in your question.

Could you please check again?
