Why one question doesn't appear next to answer when we receive form completed by users?

The problem is for a question associate with optionnal choices (yes/no).

Hi there,

Is the issue you encountered about the PDF file you receive when someone submits your form? If so, you can design the PDF submissions by using the Customize PDF tool in the submissions page.

Also, I checked your recenet forms but I was not able to find the one that contains the field with "Yes/No" option. Can you please give us the URL of the form and more details regarding your concern?
