Mobile issues: Lightbox forms jumps around; Text fields input difficult/not visible

My 'Jeron Music Quick Quote Form' as seen on http://www.jeronmusic.com/newyorkcityweddingdj has a bug when viewed on mobile (particularly the iPhone 6S where I do all my mobile tests from):

Specifically with any input into text-related fields (full name, email address, text area, etc.), the mobile screen dims to allow for text input (which prompts the spell-checker) but then you can't visually see what you're typing in as you type it.  Additionally, the quote form jumps downward, so it's not aligned, and then you have no clue of where on the form you are.  

I had a similar jump issue with the iFrame embed code for this same form, which your team solved with a ?nojump parameter.  Not sure if the same is required here., OR if this a all-mobile OR iPhone IOS specific-related issue (however, it seems to be fine on Droid).  

In any case, not being able to keep the mobile version form aligned in one place and not being able to see what you're typing for text-based fields, as you're typing it, increases chances of input errors and quite obvious frustration.

I did check your webpage which you have shared and found that you have added your form with iframe as well as lightbox embed code. I am not sure if this is intended as you have form embedded in iFrame and at the same time you will see a link to load lightbox window for same form. Please check the screenshot below:

I would suggest you to please remove one of the embed code and see if the issue persists. Please note that lightbox embed option is not optimized for mobile devices.
