Form is not displaying the fields with prepopulated text

My form is not displaying pre-filled contact in question questions ranging from 19 to 36. (Jus so you know no all of the questions have pre-filled answers.

Also the form does not display the same field widths in Chrome (vs. Explorer).

I understand that your JotForm is not displaying the prepopulated text for some fields. Could you let us know how the text is sent to the form? If there is a direct link to fill the fields on the form, please provide us with the link so that we can take a look. Also, please provide us with the link of the web page if the form is embedded on any of your website.
