Form not found This form is disabled Error

Am I doing something wrong?  I am just trying to make a form that emails myself and the sender, but it will not display.

I have checked your account and forms. This form was contained Social Security Number field in Archive. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of this information, your account will be suspended again.  

I have deleted this field and re-activated your account. Your forms are enabled now.
