Unable to view my form

I'm trying to view the form I created but it keeps telling me it's failed. My customers even see this on my site.

I was able to open the form from the URL you have provided from both Safari and Chrome browsers. Additionally, I cloned the form to my account and tested on my side. I could not replicate the issue as well. I was able to make submissions to the cloned form.

Did you embed your form into a website? If so, please provide us the URL of the website.

For additional inspections, it would be beneficial if you could provide us the details of your problem, with details the browser you are using, with screenshots.
