Feedback button not displaying forms

    http://cardsforyou.in/ this is my website . I have added a feedback button using jot form feedback plugin, but when i click the button it ain't displaying anything.

    I have checked your website and your form and I was able to see your form perfectly as you can see the screenshot below:
