How do I remove my account from the shared account?

Hi, I need to remove access that was granted to me by an old client.  I have requested this from her multiple times but there is no response.  

The forms shared are from amelzer and I want them all removed.  

Can you do that for me?

I understand that your account was added in the other account amelzer to share their forms. Unfortunately, we cannot remove an account from the shared account without the consent of the owner or unless the request comes from the user itself.
