My form questions disappeared again.. Can´t revert to older versions

I have the same problem. I can´t add or change questions because they simply don´t appear. Now i´ve tried to get to Version 3 and it all disappeared again. 

Please help me to get my form to work again, once more and please resolve this anoying bug once and for all.

I saw that you reported similar issue on this thread https://www.jotform.com/answers/997655 regarding V4. I have checked and you are still using V4. 

However when I checked form revision history of your form I was not able to revert your form to older version. 

I also replicated same issue as my colleague on your other thread in V3. Fields can not be added to your form or clone version of your form.

Considering that this is the same issue I will not escalate your issue to our developers on this thread. However I will check with our developers if there is any update on your other thread and if there is we will inform you on that thread. 

I would suggest you to create new form and use your new form until this issue is resolved.
