Responsive Forms

For some reason the field for  "Childs Birthday" is not responsive and fall of the page when viewed on the iphone.  How do I fix this problem ?

Please make the following changes to the form to fix the issue :

1. Set Shrink to OFF of Child's Date of Birth Field under Advanced

2. Add Mobile Responsive widget to the form
