    When creating a survey is there a way to only have numerical choices to choose from limited from 1 to 5.

     If there is a possibility to answer up to 20 questions, can the ability to answer more than 5 questions be blocked?

    In the "Survey Fields" there is a scale rating field that allows for rating some on a scale:

    Limiting users to answering only 5 questions may be a bit trickier.  The easiest way I can think of would be to create a checkbox field with a list of all 20 questions.  Limit the number of selections in the checkbox field to 5 and conditionally show only the questions they select:
