How can I create a form?

I have created forms here before, but for some reason whether I am trying to create a form from scratch or exporting a form from an existing form, I cannot get past the initial setup screen. When I click 'Continue' to generate the form, it remains caught in "creating form" and a new form is never established.

I reviewed your account/username caitlinkane  and indeed there was no existing form yet on your My Forms list.

However I could not replicate the issue you reported when I test creating a form using the Form Builder.

Can you please retry creating a form again. Make sure you were logged in to your account already at https://www.jotform.com/login and create a form.
