I've done the above but when I tried testing the form on my facebook page this is the error I received:
Insightly Integration Error!
API User does not have access to Lead.

I see your integration is set to Lead. If you're getting an integration error, it's most likely that you have not enabled Lead Management on your Insightly account.
To do this, log in to your Insightly account and go to System Settings > Lead Management
On the next page, just mark the checkbox and click the Save Settings button
Once done, this should allow your integration to accept leads.
