I have tested your embedded form and I was not able to replicate mentioned issue. I successfully uploaded .jpg file and .mp4 file. I was also able to submit your embedded form and I got redirected to Thank You page.
I have also checked and my submission was successfully sent and it is shown on submission page of your form.
Were you able to resolve your issue? Or maybe it was temporary issue.
If the issue still persist on your end, please let us know which device and browser are you using.
