OK, our problems started with Jot forms not delivering the PDF to our Drive.
With your assistance and advice, I deleted the Google integration and added it again ( and changed the folder in Drive to make it easier to monitor ) -- This is now working now BUT, the client NAME field is no longer used in the PDF to identify the form. In it's place, the form date stamp is used. This is causing additional work my side and I really need to revert back to the NAME field asap. I made no alterations and cannot understand why it is doing this -- kindly advise.
[ The client can select off the form if he/she is signing up as either (1) a private person or (2) a company... This should make NO DIFFERENCE in the naming of the PDF form that is delivered to the Google Drive folder that I stipulated - your last mail to me suggested that it may be a problem but I do not see how or why. ]
IN ADDITION... my email notifications ( at least the one to my email@example.com ) is now not working at all ! - So I have lost some subscriptions and although it appears correctly in the Jot Form Mail setup window, it does not work -- I suspect the clients will also NOT get a copy !!!

Please reintegrate the from with the following setting displayed in the screenshot.
I tested it and my end and it worked fine.
