How do I set certain reply emails to certain employee time request off forms?

I'm creating a Employee time request off form, and would like certain requests to go to certain people. How do I set this up? 

Email notifications can be sent conditionally based on selections made in the form:

This will allow for sending certain requests to specific addresses.
