I am only using 4 forms. 3 that are enabled and 1 that is disabled. The app will not allow me to enable the 4th form without upgrading.

I have checked your account and found you have some forms active in the "Archive" folder, what you would need to do is to disable these forms and then you will be able to enable the needed form.
You will need to do the same, disable the needed forms if you want to create or enable a new one.
