My form is disabled

When I try to preview my form it says that the form is disabled. I am using this form for my GTA V online roleplay crew on the xbox one and I believe that my account should not be suspended. If anyone can provide some insight on this incident that would be kindly welcomed.

I checked your account and found that it was automatically suspended by JotForm's phishing filter. This automated suspension happens when the filter finds fields that can be suspicious for collecting sensitive information. However, I have reviewed your forms but did not find anything suspicious. Thus, I went ahead and reactivated your account. You should be able to use your forms now.
