Form is showing as disabled but it's not.

I just created a form that was working perfectly until I tried adding a theme. Now it is showing as disabled. Please help!

I checked your account and it was auto-suspended. And that happened by our anti-phishing system that detects and disables suspicious forms. I reviewed your forms and it most probably happened because you are collecting passwords in your forms and since our forms are not allowed to be used for  such purposes our system detected them as threat.

I had deleted the password field from the form and  whitelisted your form and re-activated your account so your forms should load now.
