I followed the steps in the user guide, however, when I send the test email, the attachment is not included. Also when I try to change the email address from jotform as the sender to my email, the field does not allow me to change it.

Please note that the test email feature is simply for testing the layout of the email. You will need to make an actual submission to the form to properly test and see the file that you attached to the autoresponder.
With regards to the email, it look like you have successfully changed it as the sender email is now email@example.com
