How can I fill in information prior to sending out the same template to vendors?

I am trying to create a form where we can request approval to make changes and subsequently charge the client. I create a basic template that I would like to use, but with each form I would like to enter bits of information and then only request a signature. How can I do this?

Maybe the best option would be to use our Prepopulate application: http://prepopulate.jotform.io/

With that application you can create an URL that will prefill your form when your users click.
