Form is disabled

form says enabled on settings, but when i try to go to it, it says it is not found.  Please help.

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.   

Since your form http://www.jotform.us/form/70324429921151 is having username and password fields, the account is disabled. I have removed the guilty fields from the form and activated your account. You should be now able to view the forms normally.

Please refrain from adding such fields in your forms to avoid any further suspensions. Thank you!
