How do I put an URL into a header for a field

    We are using one of our forms so people can reserve a meeting room in our library.  We would like to have the person registering for the program to be able to link to the the policy for our meeting rooms and check a box indicating they understand the rules and will follow them.  I can't seem to figure out how to provide a link to the PDF for the policy on our website.

    It is the last question on that form.

    You need to insert a Text field to your form and put the URL link. Here's how:

    In the Form Builder, click the Add Elements button. Drag the Text field to your form. Click the Edit Text button. Highlight the text and then click the Insert/Edit Link icon. Put the link in the URL field and then click OK. You may also type the URL directly. Here's a screen capture:
