The date and time field does not fit in the frmae anymore. So when looking on a I phone you need to scroll to the right in order to see it.
In addition, my auto responder and email output map does seem to work anymore. It seems to work ok sometimes when viewing the email on an I phone. But when looking on a desktop on Chrome or Edge it does not show up. It use to work on all of them.
It also seems impossible to get back to the original commands that I set-up for the map to show with the auto responder and email notifications.Page URL:

The form can be made mobile responsive by enabling the option "Make this form responsive" in the Form layout section from Advanced Form Designer. This should enable the form fields to display correctly in the mobile devices.

Please try adding the mobile responsive widget to the form.
It should be displaying the date field correctly on mobile devices. Also, please take a look at the following guide that can be helpful in making mobile friendly forms.
