Why my form is disabled?

a few days ago I created a form, but now I get a notification "This form is disabled."

I think it's some mistake, can you please fix it so I continued to use your products.

Your forms and account were suspended because you have created forms that violates the Terms of Use.

You cannot use the form to capture credit card account data. This is considered a phishing method and is illegal.

You have to comply first to the terms and remove all components on your forms that violate the terms before we can request for consideration to reactivate your account and forms.
