Hi, my form is disabled for no reason. Can someone assist asap please?

I have checked your form and found you're asking for credit card information, do note that asking for this and other sensitive info goes against our terms, here's also a screenshot of the guilty fields:
I have removed them from your form so you avoid it to get disabled again, please also take a look on our terms so you know what you cannot ask in your forms: http://www.jotform.com/terms/
If you need to collect payment info then you must use our current payment integrations, there're some of them that will allow you to charge your customer on a later date and get the payment info, please check on this guide for more info: https://www.jotform.com/help/292-How-to-Enable-Payment-Authorization
