Nonprofit Discount Request

Hello,

I am attempting to fill out the form to request a discount because we are a non-profit organization. The form will not let me upload our 990 document. It doesn't attach anything and wont let me go any further in the process. Can you please help me? THanks! 

The upload button is working fine on the form here https://www.jotform.com/nonprofit/pricing/. I am able to upload files, even multiple files.
