Why is autoresponder email going to wrong person?

    I created a form for referrals that captures the person's name and email address sending the referral. The form also includes the referral's name and email address. Instead of the autoresponder going to the person who sent the referral, it's going to the person's email who was referred. I have attached a screenshot. In this example it's going to the privatenotes email vs. Lathea email.

    I have checked the email auto-responder and it is currently going to the referral's email address, this is how the email is setting, but you can change it by opening the emails wizard and editing the email auto-responder: 

    Once in the email auto-responder wizard go to the recipients tab and select the correct form field where the email should be sent to:
