I have checked the form shown in your screenshot and noticed it was suspended by phishing suspicious, I have reviewed that form and other forms under that account, I can see they're clear so I have reactivated your account, your forms should be loading fine now.

Please, also take a look to our terms so you know what you cannot ask in your forms and avoid getting suspended again: http://www.jotform.com/terms/

If you have any question, let us know.

And one more question: this form is under my collegue’s account. Because 2 years ago, he first prepare it and then i cloned it, and worked on it. As you will see there are many questions under that form.
So in order to build it again, i prefer to clone and work on it.
Can i export it to under my account? My colleque account name is: oozdener, my account is ozgeozek

I've checked your form and I can see it's loading without any problem, your account has not been suspended now and the form seems to be active:

I can not use (preview) old forms that I cloned from last year Screenshot 20

To avoid your forms getting suspended, please make sure you're not asking for info that goes against our terms as suggested above, otherwise your forms and account will be suspended due to violation of our terms.

You may also try loading this link to your form and see if this works on your end: http://www.jotform.com/form/70432529701350

Regarding to your second question, you will be able to clone your colleague's form by following this guide: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL

If you have any question, let us know.
