Error uploading files

    My form is:

    I have used V4 and V3 and still getting this error.

     

    Can you let me know if I am doing something wrong?

    The issue should be resolved by now. I tried to submit your JotForm and see that there is an issue with the Thank you page URL. You may need to verify the conditions and URLs used in the form. Please check and let us know if you still experience any issues with the file upload on your forms. We will be happy to take a look again.
